COVID-19 Vaccine and Dental Implants

As of this writing, it is safe to have the Covid-19 vaccine within a few days of having a dental implant. The Pfizer or Moderna vaccine should not have any affect on the tooth implant or bone graft procedure.

Although there is no proof to this statement at this time, my dental colleagues and I have seen no dental reactions or complications to the covid vaccine.

Should I wait to get the Covid vaccine until after the implant?

If you are offered the vaccine, I would take it right away. I have personally had the vaccine as well. All I experienced was a sore shoulder. The implant can even be placed on the day of the covid vaccine if you feel normal. If you feel ill, it would be best to rest and have this another day.

The vaccine should not affect your ability to heal after dental surgery. Also, the COVID-19 vaccine should not affect the implant or bone graft procedure. I recently did full arch replacement on 5 patients who had the vaccine within 5 days of my procedure.

Second vaccine dose and dental surgery

Keep in mind that the second dose is has more side effects than the first.  This is especially true in younger people.

OTC pain relievers, such as Ibuprofen and Tylenol, might weaken your immune system’s response to Pfizer or Moderna. It’s best to avoid them before you get your shot. But once you’ve received the vaccine, it is ok to take and PTC pain pill. The point is to avoid them before the surgery.

I might avoid ‘major’ dental treatment within 48 hours of the second dose.  Your body is already mounting a large immune response.  Adding a major surgery is asking a lot!

Hopefully in time we will learn more and validate this information. We all have so many questions about safety. We are figuring out life after the pandemic.

Stay safe. I will keep updating this post if anything changes.

  1. Hello,
I got my Pfizer booster shot last Saturday. It’s been 5 days and I’m still in pain – gums area (I had a few implants 3 years ago). Chewing food is not the problem, it just feels like gums are swollen a bit and pain doesn’t go away – Ibuprofen and Tylenol don’t help. Any suggestions? Thanks

Please do advise me if its ok to have my tooth extraction a day before my 2nd dose of pfizer vaccine, and to take anti-biotic for wound treatments resulting for that extraction for 7 days also with pain relievers?

    Thank you

  3. Hi Doctor, I had my pfizer second vaccination on 29th July and had lot of side effects. Now my wisdom tooth is paining a lot and my jaw bone is very much swollen and left side of my cheek too is swollen. My doctor suggested an extraction few months back which I couldn’t do at that point due to some personal reasons. So now is it safe to get the tooth extracted with all the side effects I had after vaccination? Will my immune system work well in healing post the surgery?

  6. Hi Doctor, I hope all is well. I had my 1st dose of “pfizer” yesterday morning and last night I experienced severe toothache, (not the usual). Can this be be considered one of the side effects of the vaccine? I am currently still having toothache and groggyness right now. I hope you can help me with this. Thank you so much and God Bless.

  7. Hello, please help! My dad has had smelly breath for the last 2 1/2 weeks, and when he touches his gumline it also smells. No other symptoms. He is getting his 2nd Pfizer vaccine this Saturday and has a dentist appt. for July 14th. His primary just prescribed a second dose of Amoxicillan to be safe (total of 2 weeks). Is it safe to wait until July 14th for the dentist appt? I am also very concerned about the second dose and cases of myocarditis. I read somewhere that an infection may cause this. Could a gum infection trigger myocarditis if he gets the second vaccine without getting it treated first?

    • It sounds like your dad has some pretty bad dental disease. He should have both the vaccine and his dental work done as soon as possible. Sounds like he has periodontal disease. Myocarditis is not a common effect.

  10. Doctor,
I appreciate your article above. I want to share with you my experience. I have had considerable dental work performed recently. The week of Christmas, both upper cuspids were extracted on an emergent basis due to infection. Eight weeks ago I had all remaining upper teeth removed and bone grafts implanted. My dentist and periodontist refer to the procedure and work done as extensive and major. I’m not sure why but I do know that they viewed my infection history, proximity to the sinuses, deterioration of the bone and condition of teeth combined with the procedure and bone grafts as extensive work that would require time to fully heal. They were correct. One month later I went and got the J&J vaccine. Within 30-45 minutes I had a sudden and severe headache and felt really tired until midnight of the next day. Then 36 hours after the vaccine, I began to hurt all over, my head, my neck, my joints, my legs, my stomach. I had a terrible headache, my stomach was queasy. I became nauseated. I got dizzy just sitting. I had no energy at all. In short, I felt very very sick.

So I had an immediate reaction to the vaccine, indicating that I had already had Covid. I suspected that I had it. Back in the fall. Around October. It lasted two weeks. My symptoms at that time were similar to the ones I’ve experienced since taking the vaccine. So I assumed, Ok, I must have had Covid. These symptoms, though miserable, will be gone in a couple of days.

That is not what has occurred. In three days, it will be 4 weeks since I had the vaccine. I continue to battle fatigue, stomach upset, headaches. Though it seems to be slowly improving, I experience taking 2 steps forward and one step back. I get weak easily. And battle fatigue daily. Much exertion at all and can feel as bad as I did 4 weeks ago. At times I think it is leaving. Other times I think it never will.

My dentist and periodontist saw me at 6 weeks post procedure and were extremely pleased with my healing, even pleasantly shocked at my rate of healing. They believed that my symptoms are related to the vaccine and that I most likely had Covid, given my immediate response. I contacted the pharmacist at the pharmacy where I received the vaccine. He believed I had Covid as well but my symptom severity and duration is definitely outside of any experience of others who had the vaccine that he was familiar with. I contacted Johnson and Johnson on the 10th day post vaccine. They documented the 10 day experience at that point and were concerned about my pattern being similar with others who had blood clotting issues. That doesn’t appear to be the issue. They too said I must have had Covid or my reaction time after getting the shot would not have been immediate. (Less than an hour)

I am 63 years old. Given that I am definitely an outlier. It has been and continues to be quite debilitating. I have confidence that it will eventually resolve. I communicate this because after searching I find nobody that seems to be aware that this can and is occurring. I discovered only today another person who is a friend of a friend, who also had Covid, got the vaccine and after a month is still unable to carry on with a normal daily life.

Given what I’ve been through, I would not recommend that somebody not take the vaccine surrounding major dental work. But I definitely would not advise that they get the vaccine if they have already had Covid. And if unsure, I would recommend getting the antibody test prior to getting the vaccine. Because these experiences are occurring. And they are not insignificant. I fear that medical professionals are erring in the side of caution. And in doing so, they are erring.

I am not a physician, not clinical at all. But have worked in healthcare for 35 years. As a healthcare executive. I have seldom been sick. But this is one of the worst experiences I have ever had. Otherwise I would not take the time to write this. My instinct says that my immune system was already revved up due to healing related to the dental work. Also, I had Covid. I got the vaccine. And it sent my immune system into orbit. And it continues in overdrive. Possibly because I am still healing from the dental work and bone grafts.

I would like to hear your thoughts on this. And any suggestions you might have would be very welcomed.

    Thank you,

    Jamie Townsend

    • It is certainly possible that you are an outlier on the bell shaped curve!

That being said the vaccine is a wonderful gift to all and is helping bring an end to the pandemic.

You dismissed Jamie’s very detailed email to make a joke about a Bell curve and promote propaganda about a “gift” that may be an attempt at eugenics for all we know so far via freedom to information. I had two teeth removed yestersday. I had a bad flu in March of 2020. I’m deciding to postpone it so I can heal my sockets over. Comment? Make a joke. Say something profound.

  15. Hello Dr. Amin,

I’m currently scheduled to receive my second dose of the Pfizer vaccine on Monday, May 24th. I also have a wisdom tooth removal scheduled for the next day, Tuesday, May 25th. They will be removing both of my right side wisdom teeth as the bottom tooth is impacted. I will be getting IV sedation for the wisdom tooth removal and probably the standard post-procedure prescription painkillers. I was wondering, is it okay to go through with this? Or could there be any issues from doing these things on back-to-back days that would make me want to reschedule the vaccine/wisdom tooth removal? Thank you in advance for any help/information you can provide!

I didn’t expect to feel like this and the tooth pain totally caught me off guard. The CDC needs to add this to their list. I see I’m not the only one experiencing this.

My 2nd vaccine shot is scheduled on 05/19, should i go ahead with the 2nd shot or should i postpone it? I am wondering if azithromycin would reduce the efficacy of the vaccine. Pls clarify.

  26. 40 hours after second Moderna shot and after a day of flu symptoms I woke feeling much better. Until I brushed my teeth – pain. Then I took a drink of coffee – intense tooth pain. Permanent front upper bridge pain is the worst here. But bottom front teeth also ache. I’ve never had any tooth pain before. Since others have had same issue after their second Moderna vaccine, it seems like this is a bit more than coincidence.

  30. I got my second shot of Moderna a few days ago. I’m having excruciating pain now in my teeth/gums, usually different teeth in different parts of my mouth. Sometimes, I could not even eat because it hurts too much to chew. I have TMJ periodically and what I am experiencing now reminds me a lot about it but it has never been this bad and I’m not sure if it’s TMJ this time. Could it be related to the vaccine? The pain at times becomes unbearable, at other times it’s moderate or disappears unless I start to eat. I brush my teeth a lot, use my water pick a lot and and electric toothbrush and also I floss a lot. That seems to help somewhat for a little while. I tried to wear my night guard but most of the time the pain increases. Thank you in advance for your answer.

          • The pain is less now, but it’s still not gone, much better though. I’m hopeful it will go away and after reading the posts of the other people experiencing similar pain it seems to me that it may have something to do with the vaccine. I don’t have any regrets about getting the vaccine. I just hope that all the tooth/gum pain will totally go away.

      • Hi Dr. I’m actually going through a very similar response to my 2nd dose of moderna as Rossie. The night that I got it I had the worst tooth sensitivity and pain in my whole mouth. It was so hard to fall asleep. I had other side effects through the night and the whole next day. The tooth pain went away the next morning. And now three days later I’m having another headache and tooth pain throughout my entire mouth in different areas. Although I have TMJ this is different and I rarely have sensitivity/pain like this.

      • I was very surprised to see this post. I too have had both doses of the Covid vaccination and have been experiencing the same discomfort in my teeth and gums for more than a month now. I also have experienced a metallic taste sensitivity to my teeth especially when I eat anything sweet. I’ve had numbness in my jaws and lips and tingling in my hands. Initially suspected TMJ but believe it’s links to the vaccination. I spoke with someone over the weekend who is also experiencing the metallic taste and teeth sensitivity. Very interested to learn more about this odd side effect.

    • Rossie,
So glad I found this. I am having the same problem. I’ve been searching all over and nothing until now. My pain started about 4-5 days after my 1st dose and was really bad for about 4 days. So bad I could barely brush my teeth in the area or eat on it for two days. It began to hurt in an area where I had a cavity done and has been sensitive for about a year and in upper and lower areas not he left side. This morning it’s a little sensitive but barely any pain! I think this is definitely a side effect of the vaccine.

        • I just wanted to share that I believe there has to be SOME sort of link between the intense tooth pain and Covid-19 but not necessarily the vaccine. I tested positive for Covid-19 almost 4 months ago and about 4 weeks ago I was having the symptoms many on here have been describing. Excruciating tooth pain that would migrate to different teeth over a period of several days. I had intense pain in my upper back tooth (which I am scheduled to have pulled due to cavities) for a couple hours and then it would switch to my lower back tooth (which is chipped but has never caused any pain) and an hour later I was having headache pain on my left side(like I was being stabbed with an ice pick in my temple) …but no tooth pain.,but a few hrs later the tooth pain was back!! I felt like I was sounding like a hypochondriac . It was like NOTHING I have EVER experienced!!. I truly believe that there has to be some correlation between Covid-19 virus and tooth/head pain. In my case it would have been due to having the virus PREVIOUSLY. As for the vaccine, however, I got my first dose of the Moderna VACCINE one week ago and have not had any tooth pain whatsoever…just a sore arm and mild sinus headache. I’m curious whether anyone who reported having this tooth pain after getting the vaccine previously tested positive for the virus! ??Sounds like someone may need to do a study on this🧐

        • I took my 2nd Pfizer dose yesterday and now I have a level 7-8 pain in one of my teeth, but all/most of them are more sensitive than usual. Came online to see if others had the same. I’ve never had this kind of sensitivity before. Can’t imagine it’s from anything other than the vaccine. Hopefully, it goes away like Adrian posted.

      • Hi- so weird I found this. I’ve been having some random tooth nerve pain after vaccine, as well. First one was felt 3 days after 1st shot – not related to eating or drinking but intermittent and in a tooth I’ve never had issues with. Next morning gone and hasn’t returned. I’m now 2 1/2 weeks post second shot and now suddenly having intermittent tooth nerve pain in the front of one of my front teeth where I had a filling repaired 4 months ago. So odd. I never had pain after my filling was done. I just find this all pretty strange post-vaccine because these tooth nerve pains are brief and fleeting.

      • I was just looking up whether the second dose of the Covid vaccine causes dental pain and found this. At the start of this week, a root canal was attempted on a tooth that was causing pain. I was prescribed antibiotics before coming in and the pain was mainly gone. The root canal was not successful because of where the cavity was located and laxk of enough structure. I was given a temporary fill and a referral to an oral surgeon. Aside from the first day, my mouth/tooth have been fine. This morning, after receiving the second dose of Moderna, my tooth is excruciating. It hurts like it did before antibiotics, and when I accidentally got a piece of food stuck inside. The pain is radiating in my jaw. Don’t know if the soreness/inflammatory response from the vaccine is triggering it again, but it was fine for days (and should be better than before seeing the dentist) and now I’m miserable.

My 18 year old sons are scheduled to have their 2nd Pfizer shot on April 27. They both need to have their impacted wisdom teeth removed with anesthesia (via IV I believe). How long would you recommend they wait to schedule their extractions after the second dose of Pfizer? Also, how long would you think they would experience pain and swelling after a non-complicated impacted 4 wisdom tooth extraction? Thank you so much for any input you are able to provide.

    • If it is possible to wait for a while then by all means wait. It is normal to experience some pain and swelling even after non-complicated wisdom extractions. Do not assume both of your sons will respond the same way either. If there anything like my kids they are all very different…LOL

  42. Sounds like I have a lot of the same questions as everyone else but I want to be sure so here it goes. My 18 y/o son just had his wisdom teeth out on 4/16, 4 days ago, and he is taking Methylprednisolone 4 mg dose pack for 6 days with the last dose finishing tomorrow, 4/21, AND he is also taking as prophylaxis only, Clindamycin HCL 300 mg which he will be finishing on 4/22. He is also seldom taking the Ibuprofen 800 mg for pain which is only like a 4 out of 10 for him. He will have no problem holding off on Ibuprofen the day of his vaccine. He was scheduled for his first Moderna COVID vaccine this Saturday, 4/24, in the afternoon. Are these ABs and Steroids going to affect the efficacy of the vaccine? Should he postpone his first dose and if so for how long do you recommend? Thank you for your help and for this thread.

I am also a COVID survivor, so the vaccine side effects are notable.

My 19 yr old daughter is to begin a 6-day Prednisone pack the day before wisdom teeth extraction (4 total), with a Diazepam tab the morning of, and some anesthesia. She is seeking a Covid Vaccine one month before. If Pfizer, the 2nd vax shot would be 6 days before the Prednisone begins. Is this unwise?Or should she wait a few weeks to seek 2nd Vaccine shot. • Yes, I got my first dose of the Moderna Vaccine on April 15 and 2 days later my bottom left side became swollen and my teeth were sensitive to cold. I had my dentist look at my teeth about 4 months ago and everything was fine .I think this is due to the moderna vaccine. It is 3 days since i got the vaccine and it seems to be getting better. But still teeth on bottom left are sensitive and my face is swollen on the left side ,but not to bad. Any ideas?

As a reply to the post regarding dental pain following injection. Following my first moderna vaccine, I developed a painful jaw and swelling near an implant. Following my second vaccine, a couple of days ago, I experienced the same swelling and pain near a different implant but this time, I seemed to have developed an abscess.

I am getting my 4 wisdom teeth removed on April 23rd and I am pretty sure I will take anesthesia with IV. I was planning on getting my first dose of the Pfizer vaccine on April 18th. Is this timing okay? This means my first dose would be 4 days before the surgery and I would be getting the second dose about 2 weeks after the surgery. Should I wait to schedule vaccines after the surgery, or should I carry on with this plan?

  63. Hello Dr. Amin,
First of all thank you so much for the great informative article and answering every question here and helping us all! 🙏🏻

I am very confused about which vaccine to take – do you prefer Pfizer over JnJ?
My n my husband got covid in Oct’2020 and have read in few articles that since we already have some antibodies due to covid no need to take two dose vaccine hence more inclined towards JnJ.

My second question is my hubby is type 2 dib and he recently started metamorphic medicine for couple of weeks and his Pfizer 1st dose shot is this Saturday, is he good to go for the vaccine and keep continue with his dib medicine.?

My last question is I am having cavities in 3-4 teeth and May be one need a root Canal the dentist is not sure, she said while cleaning the cavity I will let you know if root canal is required or not as of now in x Ray the nerves are not touching but it’s v close, for the cavity filling they will use the numbing injection- is it ok to get the treatment done and also if root canal required then antibiotics etc be given… I am also planning for Pfizer first dose this Saturday (10th April 2021). Should I wait for my dentist appointment and get it done after both the doses or can it be done in between the doses also?

Please help replying to my queries.

Thank you so much in advance for giving your valuable time in answering our queries. 🙏🏻🙏🏻

As for preference of Pfizer versus Johnson & Johnson versus Moderna… That’s not something I can really comment on. I think they are all excellent vaccines. I personally had the Moderna vaccine.

I think both of you should get vaccinated sooner than later! The vaccine is the beginning of the end of this pandemic!

Work still in progress. Root canal on 6. Had temp. # 7 crown chipped off in back. Noble metal exposed. Clindamycin 15 days. Cause other painful problems in mouth and facial skin. Off med, finally feeling decent. Waiting to 4/14 for # 7 root canal and 4th recrowning on it! Extraction sounds good to me!
2nd Covid Pfizer 4/2 Friday noon. By evening those two teeth went into pain all over again.
Doing what I can for relief until I get dentists or endodontist Monday.
Don’t believe 2nd time tooth pain (same area)with Covid vaccine is coincidental! It seems to attack (my) weakest points.
Including arthritic knees. 1st time one knee locked up few days after vaccine. Went to Orthopedic for knee shots.
Still debating if conincidental … or, if this should be reported to Covid manufacturers as another side effect…
As always Dr. Amin, your expert opinion is appreciated. My dentists don’t want to get involved with this Covid and teeth/ gum issues.

Thankful I did not have to take any other pain medication following the procedure.

  102. I am getting full upper permanent dentures. On Monday the 22nd, I’m having all of my upper teeth extracted and 6 implants placed. The next day, March 23, I’m receiving the 2nd dose of the Pfizer vaccine. My dentist told me that it won’t be a problem having the vaccine the day after the procedure. Do you have any information that shows that there could be problems with this?

    Reply
    • This depends on what type of full arch dental implant replacement surgery you are having done and the method. In my practice my patients virtually have no pain. Part of the reason is they are given intravenous steroids and Advil-like medications. These will both slow down your immune response to your second dose of the Pfizer vaccine. I do not think there is a way to avoid any pain medication before your second dose. If it were me I would try to rearrange 1 of these appointments.

      • Thank you for the advice. I’ll speak with my dentist tomorrow. I know he plans on giving me Motrin to take for pain. Since that will slow down my immune response to the vaccine, I will reschedule my appointment.

Tomorrow morning 03/17/21 I have a dental appointment for the follow up of my implant. It’s been healing since January and is all healed up now they are going to cut my gum to get ready for the tooth piece. I’m not sure of the correct terminology but they are going to definitely make a new opening. But is everything going to be okay? I’m 26 years old by the way.

    1. When should I stop taking Tylenol before my vaccination?

    2. Would it be alright to have my tooth extracted, say on March 16th?

    3. Would it be better to have my tooth extracted on the 20th or 21st or should it be a later date after the 19th?

I am 33 yrs old, no health issues, and would really appreciate your insights. Thank you.

I thought it was my jaw initially, but I am not sure anymore, I am seeing my dentist in 2 days, but can this be related?

          • Oh gosh 😩 the dental pain is likely coincidental rather than the cause. Just bad timing ⏱

See your dentist and let us know what was the problem. I would appreciate the follow up.

          • Hello Dr. Amin,
I’ve just read through a fair amount of these questions about the Covid Vax and getting dental procedures.
What struck me is that there are quite a few people talking about dental pain/problems after the vax and your response to them was that it was coincidence.
Perhaps you would find it helpful to do more research on what side effects people are experiencing. Note, however, it is difficult to find these reports on main stream media sources.
Thank you for a wonderful website!

          • I HAVE REPORTED IT! For sure! But so far there is still no relation being reported. I have personally seen at least 1000 vaccinated patients and have not seen a case where there was not a dental reason for the pain.

I am just relaying the truth as that is my obligation as a doctor.

            I really appreciate your comment!

          • The problem is managing pain. I’m sorry I did not get to you with such short notice by March 23. What did you decide to do? I had the Moderna vaccine as well.

The issue is that AlloDerm does cause significant pain so you’d end up having to take some ibuprofen. I myself try to avoid the ibuprofen both before and after. The first dose is really nothing so I’m sure you will be fine either way

A lympf vassel on my neck appeared and a Doctor said it must be from few pimples near that I have extracted. I was prescribed biprofenid 150mg 2xper day for 10 days (ketoprofen). To be safe I made a panoramic photo of my teeth and got a dentist to look into it. He said the vessel can be from a bottom tooth with a crown. He said it must be opened, shift removed, roots cleaned and a new crowned placed. Few doctors told me its not safe to have the vaccine. That I have to be fully healthy to get it. Could you offer some inside and advice? Thank you!

But now it’s nasty… one side is so much worse than the other. It is still abnormally swollen and so much pain… I guess because I had the impacted molar… I had fever and couldn’t really sleep these few days… I just hope he didn’t wreck the nerve or some other teeth as well…

I am going in for check up tomorrow, but I will see if I can get in today.

I am still hoping that it would get better by Wednesday, otherwise will have to reschedule the vaccination 🙁

I am very anxious due to being early 70’s, having high no, hypertrophic cardiomyopathy and Afib and 6 different medications and mostly fear having to go off Eliquis for 2 days while continuing to take 81 mg aspirin daily. Combined with unknowns about how the vaccine is going to affect everything I have almost cancelled appt. i suppose I am afraid of looking foolish by continuing to express fears to the doctor/ oral surgeon. That is what led me here because I googled what my concerns were!

I’m not sure I understand what it means to put implant too deep or shallow? When I look at photo and see more tooth area it looks to me like it’s because there is less gum there.
